EACH applicant to Chicago Crimson Moon is REQUIRED to truthfully fill out this form before they can be considered to attend a party, or to become a member of the club. For the safety of our guests, your real identity WILL be verified (you may keep it a secret from other guests if you wish, but Crimson Moon management has to know).

APPLICATION TO ATTEND CHICAGO CRIMSON MOON PARTIES
All information provided will be held in strictest confidence. Our mailing list is not sold or traded to anyone. We will exercise discretion in all our mailings to you, and expect the same in return.

BY MY SUBMITTING THE ABOVE INFORMATION, CONSIDER MY ACTION AS A LEGALLY BINDING ELECTRONIC SIGNATURE, I HEREBY HOLD HARMLESS CHICAGO CRIMSON MOON, ITS STOCKHOLDERS, EMPLOYEES, CONTRACTORS, FRANCHISEES OR BOARD MEMBERS FROM ANY LOSS, DAMAGE OR INJURY WHICH MAY OCCUR AS A RESULT OF MY ATTENDANCE OR PARTICIPATION IN ANY EVENT SPONSORED OR HOSTED BY CHICAGO CRIMSON MOON. I ALSO CERTIFY THAT I HAVE READ, UNDERSTOOD AND WILL ABIDE BY THE FAQ AND CODE OF CONDUCT, WHICH ARE EXPRESSLY MADE PART OF THIS CONTRACT, WHICH GOVERNS ALL THOSE WHO ATTEND CHICAGO CRIMSON MOON PARTIES.
