Thank you for your interest in attending a Crimson Moon spanking party.  Before
you can do so, you must answer these questions and mail a SIGNED copy to us.  
 
EACH applicant to Crimson Moon is REQUIRED to truthfully fill out this form 
before they can be considered to attend a party, or to become a member of the 
club.  For the safety of our guests, your real identity WILL be verified (you 
may keep it a secret from other guests if you wish, but Crimson Moon management
has to know).

list is not sold or traded to anyone.  We will exercise discretion in all our
mailings to you, and expect the same in return.

STOCKHOLDERS, EMPLOYEES, CONTRACTORS, FRANCHISEES, OR BOARD MEMBERS FROM ANY 
LOSS, DAMAGE OR INJURY WHICH MAY OCCUR AS A RESULT OF MY ATTENDANCE OR 
PARTICIPATION IN ANY EVENT SPONSORED OR HOSTED BY CRIMSON MOON.  I ALSO 
CERTIFY THAT I HAVE READ, UNDERSTOOD AND WILL  ABIDE BY THE CODE OF CONDUCT 
WHICH GOVERNS ALL THOSE WHO ATTEND CRIMSON MOON LTD. PARTIES.
